The Complete Microneedling Aftercare Guide
Microneedling works by creating controlled micro-injury to the skin, then letting your body's own repair process build new collagen. That means the 72 hours after your session matter almost as much as the treatment itself. Skip a step here and you can blunt your results or trigger a complication that was entirely avoidable — sun exposure being the most common offender.
This guide walks through exactly what to do, hour by hour and day by day, based on the protocol we use at Wiser Health.
Quick Reference: Your Aftercare Timeline
| Timeframe | Do | Avoid |
|---|---|---|
| 0–6 hours | Nothing but the products sent home with you | Makeup, active skincare, touching your face |
| First 24 hours | Gentle lukewarm cleanse only, barrier/healing products | Retinoids, AHAs/BHAs, vitamin C, niacinamide, gym, sauna, sun |
| 24–72 hours | Broad spectrum, 100% mineral SPF 30+ any time outdoors; reintroduce normal products once redness fades | Picking or forcing off flaking skin |
| Days 3–7 | Resume actives only once redness has fully resolved; makeup okay after 48 hrs | Exfoliation, aggressive product layering |
| Ongoing | Daily broad spectrum, 100% mineral SPF 30+, no exceptions | Significant sun exposure in the week before your next session |
If you remember nothing else: broad spectrum, 100% mineral SPF 30+ sunscreen is not optional. Your skin's absorption rate is elevated for up to a week post-treatment, and UV exposure during that window directly undermines the collagen response you just paid for.
Immediately After Your Session (0–6 Hours)
Your skin may look and feel like a mild sunburn — redness, warmth, some tightness. This is expected and it's the point: it's the inflammatory signal that triggers collagen remodeling.
What this isn't: Aggressive, maximum-trauma protocols built for visible drama can produce heavy bleeding and days of raw, angry-looking skin. That's not how this protocol works. Some pinpoint bleeding can happen during the procedure, but it's minimal and self-limited — it's not typical of every session. Redness and warmth are usually mild too; most patients are the only ones who notice anything by a few hours post-treatment. When flaking happens, it's fine and subtle — not the sheet-like peeling of an actual sunburn — and a pure hyaluronic acid serum plus a ceramide-rich moisturizer in the days after treatment minimizes it further.
What to do
- —Apply only the products provided
- —Cleanse with lukewarm water if needed — no foaming or exfoliating cleansers
- —Keep hands off your face
What not to do
- —No makeup
- —No active skincare — no retinoids, no vitamin C, no exfoliating acids
- —No touching your face with unwashed hands
This isn't precaution theater. Your skin has thousands of microchannels open right now, and anything you put on it goes in deeper and faster than it would on intact skin.
The First 24 Hours
This is the window where most avoidable mistakes happen — usually because someone feels fine by hour 10 and decides the rules don't apply to them anymore. They still do.
What to do
- —Gentle, fragrance-free cleanser, lukewarm water
- —Apply only barrier/healing products
What to avoid entirely
- —Retinoids, AHAs, BHAs, vitamin C, niacinamide — all of it, for the full 24 hours minimum
- —The gym, sauna, steam room, or anything that makes you sweat heavily
- —Direct sun exposure
Why the sweat restriction matters: sweat on freshly microneedled skin can introduce bacteria into open channels and increase irritation. Skip the workout. It's one day.
24–72 Hours: Redness and Sensitivity Continue
Redness and mild swelling may persist through this window — that's normal, not a red flag. You can begin reintroducing your regular skincare routine, but only once the redness has actually resolved, not on a fixed clock.
Non-negotiable: broad spectrum, 100% mineral SPF 30+ any time you're outdoors, full stop. This is the single highest-leverage thing you can do to protect your results.
- —Don't pick or peel any flaking skin — let it come off on its own
- —Don't restart actives before redness is gone, even if it's “just been a couple days”
Days 3–7: Reintroduction Phase
By day 3, most people are past the visible redness stage.
- —Makeup can be resumed after 48 hours
- —Actives (retinoids, acids, vitamin C) come back only after all redness has fully resolved — not on a fixed schedule, based on your skin's actual state
- —Continue broad spectrum, 100% mineral SPF 30+ daily
- —No exfoliating and no picking at flaking skin, even if it's tempting
Your skin's product absorption is significantly elevated for up to a full week post-treatment. That's an advantage if you're using it deliberately — it's a liability if you layer on something aggressive too soon.
Between Sessions: Ongoing Maintenance
Microneedling results are cumulative. Each session's full benefit continues developing for 4–6 weeks as new collagen matures — you won't see the complete picture from a single treatment, and you shouldn't expect to.
Daily, non-negotiable: broad spectrum, 100% mineral SPF 30+. This is the one habit that determines whether your results compound or get erased.
- —A topical retinoid at home, if appropriate for your skin — ask at your next visit
- —An antioxidant serum (vitamin C) in your morning routine
- —Avoiding significant tanning or unprotected sun exposure, especially in the week before your next session
When to Contact the Practice
Most of what you'll feel post-treatment is normal and expected. Contact us if you notice:
- —Redness persisting beyond 5 days
- —Signs of infection — pus, increasing warmth, fever
- —An unusual rash or breakout pattern that doesn't match typical post-treatment sensitivity
- —Anything you genuinely can't characterize or that feels off
I'd rather hear from you and rule something out than have you wait it out unnecessarily.
Frequently Asked Questions
How many sessions before I see results?
Radiance and texture improvements are typically visible by session 1–2. Meaningful change in fine lines, scarring, or pigmentation generally requires 3–6 sessions, spaced monthly. Scarring and melasma are slower responders and often need 6 or more. We'll talk through realistic expectations and adjust as we see how you respond.
Can I wear sunscreen the same day as treatment?
No — for the first 24 hours you're avoiding all products except what I provide, and that includes sunscreen. Since you can't apply SPF during that window, stay out of direct sun entirely, and wear a wide-brimmed hat if you need to be outside. After the first 24 hours, broad spectrum, 100% mineral SPF 30+ is required daily, indefinitely.
Why can't I use my retinoid right away?
Retinoids and other actives on freshly needled skin increase irritation risk and can undermine the barrier repair process you actually want happening in the first 24–72 hours. The wait is short and it protects the result.
Is flaking normal?
Some skin flaking or dryness in the days following treatment is expected. Let it resolve on its own — picking or exfoliating it prematurely can cause irritation or uneven healing.
What if I have a history of cold sores?
Tell me before your appointment. Microneedling near the mouth can trigger an outbreak in patients with a history of oral herpes, and we'll discuss precautions in advance.
